MEDICI BESPOKE CONCIERGE HEALTHCARE SERVICES
Terms & Conditions
These Terms & Conditions (“Terms”) govern participation in the Medici Bespoke Concierge Healthcare Program (“Program”) offered by Medici Medical Group, P.A., a Florida professional association (“Medici”). By submitting payment for any Medici Bespoke Concierge invoice, the individual(s) receiving services (“Member(s)”) agree to be bound by these Terms.
1. Scope of Services
For the duration of the Membership Term, Medici will provide Members with the following VIP Concierge Healthcare Services:

- Annual executive health assessment (on-site at Medici Executive Health Center, Austin, TX).
- Toxicity and heavy metal screening.
- Advanced genetic testing.
- Multi-cancer early detection screening.
- Unlimited concierge primary care via video, phone, and text.
- Access to a specialist network of 800+ specialists across 130+ sub-specialties (includes two consultations per Member annually).
- Customized peptide consulting.
- Unlimited nutrition consultations, personal training, and performance-based lifestyle coaching.
- Continuous glucose monitoring (CGM) twice annually.
- Comprehensive bi-annual laboratory testing.
- Full-service care concierge support.
2. Fees, Payment, and Renewal
Membership Fees

- Individual Membership: $20,000 per year
- Couple Membership: $35,000 per year
Payment Options
Fees may be paid:

- In full upon receipt of invoice, or
- In twelve (12) equal monthly installments of $1,750 (individual) or $3,083 (couple), paid in advance.
Late Fees
Any amounts not paid when due shall accrue a late fee at the rate of 1.5% per month (or the maximum rate permitted by law, if less), calculated from the due date until paid in full. Medici reserves the right to suspend or withhold services until all outstanding balances, including accrued late fees, are satisfied.
Acceptance of Terms
Payment of any portion of an invoice constitutes full acceptance of these Terms & Conditions.
Renewal
Membership automatically renews for consecutive one-year terms unless written notice of non-renewal is provided at least 60 days prior to the renewal date. Any fee adjustments will be communicated in writing before renewal.
3. Termination
Termination for Convenience
Either party may terminate Membership for any reason with 60 days’ written notice.
Termination for Cause
Either party may terminate immediately in the event of:

- A material breach not cured within 30 days of written notice.
- Bankruptcy, insolvency, or cessation of business.
Post-Termination
Any unused services may be donated to another individual at the Member’s discretion.
4. Member Responsibilities
Members agree to:

- Provide accurate and updated insurance information for services not included in the Program.
- Pay any additional fees for excluded services such as external labs, imaging, urgent care, hospitalization, or other services outside the Program scope.
5. Limitations of Liability & Waivers
Liability Limit
Medici’s total liability for any claims arising in a single Membership Year shall not exceed the total fees paid during that year.
No Insurance Coverage
The Program does not constitute health insurance. Members are responsible for costs of services outside the Program.
Telemedicine Waiver
Members acknowledge the inherent limitations of telemedicine and waive any claims against Medici or its providers arising from telemedicine-based evaluations or treatment.
6. Compliance & Confidentiality
HIPAA Compliance
Both Medici and Members will comply with HIPAA standards to safeguard Protected Health Information (PHI). Any HIPAA violation constitutes a material breach of these Terms.
Confidentiality
Both parties shall maintain the confidentiality of proprietary or sensitive information shared under these Terms and use such information solely for purposes of delivering or receiving Program services.
7. General Provisions
Governing Law
These Terms are governed by the laws of the State of Texas. Any disputes shall be resolved in Travis County, Texas.
Dispute Resolution
Disputes will first undergo mediation. If unresolved, they will be resolved by binding arbitration pursuant to the rules of the American Arbitration Association (AAA).
Force Majeure
Medici is not liable for delays or failure to perform due to causes beyond reasonable control, including natural disasters, pandemics, or labor disruptions.
Severability
If any provision is deemed invalid or unenforceable, all remaining provisions shall remain operative and enforceable.
Entire Agreement
These Terms, in conjunction with any Medici HIPAA privacy notices or intake consents, represent the entire agreement between the parties. No modification is valid unless made in writing by Medici.
Assignment
Membership may not be assigned or transferred without Medici’s written consent.
Non-Variation
No waiver or modification of any provision is valid unless in writing and signed by the waiving or modifying party.
8. Indemnification
Members agree to indemnify and hold Medici harmless from any claims, liabilities, damages, or expenses arising from:

- Any breach of these Terms, or
- Any negligent act or omission by the Member to the extent allowed by law.
9. Representations & Warranties
Member Representation
Members represent that they have the authority to participate in the Program and that doing so does not violate any applicable laws or agreements.
10. Intellectual Property
All intellectual property rights created or provided by Medici—including but not limited to trademarks, copyrighted materials, protocols, and proprietary methodologies—remain the exclusive property of Medici. Members may not use, reproduce, or distribute such intellectual property without prior written consent.
11. Notices
All notices under these Terms must be provided in writing and delivered by hand, certified mail, or email with confirmation to:
